Parental Consent Forms

When Is Parental Consent Required?

Some youth programs are required to obtain parental or guardian consent before a participant may take part in program activities. CYS determines whether a parental consent form is required based on the nature of the program and associated activities.

If a consent form is required, CYS will provide the current approved form as part of the program registration process. Programs should only use the form(s) provided by the CYS office for that specific program.

Do not use older versions, previously approved forms, or forms from prior programs. Using the current approved form helps ensure compliance with university requirements and that participants and families receive accurate and consistent information.

Completed consent forms must be collected and uploaded to the youth program registration system. Uploading forms to the registration system ensures appropriate document retention and access to required records. Programs should not maintain separate official copies in place of the registration system record. 

Who may sign?

Required consent forms must be completed and signed by the participant’s parent or legal guardian. Programs should not accept forms signed by camp counselors, teachers, coaches, chaperones, relatives, or other adults accompanying a participant.
