All consent and release forms required by CYS are signed by parent/guardians and uploaded to the CYS registration portal. Depending on program type, these may include:

  • Consent to participate
  • Travel waiver
  • Medical forms
  • Internship assumption of risk form
  • Media release

Standards of Behavior
